Mr. Smith joined Intuitive in June 1997 from Hillenbrand Industries, where he was Senior Executive Vice President. Mr. Smith joined Hillenbrand in 1978 and during his tenure he was also a member of the executive committee, the office of the president and the board of directors. Mr. Smith has also held positions with The Boston Consulting Group and IBM. Mr. Smith received his BSEE from Utah State University and an MBA from Harvard Business School.
Bob Duggan is the Founder of the investment firm Robert W. Duggan & Associates. Mr. Duggan has been a private venture investor for more than 30 years and has participated as a director of, investor in and advisor to numerous small and large businesses in the medical equipment, computer local and wide area network, PC hardware and software distribution, digital encryption, consumer retail goods and outdoor media communication industries. Mr. Duggan served as Chief Executive Officer and Chairman of the Board of Directors of Computer Motion, Inc., from 1997 until the conclusion of its merger with Intuitive Surgical in June of 2003. Prior to Computer Motion Inc., Mr. Duggan's tenure as a board member of Government Technology Services from 1986 until 1993, during which time GTSI was recognized by INC Magazine as the fifth fastest growing privately held company in America. Mr. Duggan has served as a board member of Chart House Enterprises, Inc., was a Chairman of the board of Metropolis Media and has served as a Chairman of the Board and principal fund raiser for Communication Machinery Corporation (CMC). Before it was sold in July 1988 to Rockwell International, CMC was one of the world’s top three ethernet networking companies. Mr. Duggan is a member of the University of California - Santa Barbara Foundation Board of Trustees.
D. Keith Grossman joined the Intuitive Surgical Board of Directors in May 2004. Mr. Grossman is a Managing Director of TPG Biotech, L.P., the venture capital arm of TPG Capital (formerly Texas Pacific Group), one of the world’s largest private equity firms. Previously, Mr. Grossman served as President and Chief Executive Officer of Thoratec Corporation, a publicly held medical technology firm, from 1996 to 2006. Prior to Thoratec, Mr. Grossman was a Division President of Major Pharmaceuticals, Inc. from 1992 to 1995. From 1988 to 1992, Mr. Grossman served as Vice President of Sales and Marketing for Calcitek, Inc., a manufacturer of implantable medical devices and a division of Sulzermedica (formerly Intermedics, Inc.). Prior to 1988, Mr. Grossman held various other sales and marketing management positions within the McGaw Laboratories Division of American Hospital Supply Corporation. Mr. Grossman remains a member of the Board of Directors of Thoratec, and also serves as a member of the boards of directors of Sleep Solutions, Inc., and Vibrynt, Inc., both private medical technology companies. Mr. Grossman earned his Bachelor’s Degree from Ohio State University, and his Master’s of Business Administration degree from Pepperdine University.
Eric H. Halvorson has been a member of our Board of Directors since our acquisition of Computer Motion in June 2003. Mr. Halvorson joined Computer Motion in July 2002 as a member of its Board of Directors. Mr. Halvorson is currently President and Chief Operating Officer of Salem Communications Corporation. From 2000 to 2003 and 2005 to 2007 he was a Visiting Professor of Business Law and Accounting and Executive in Residence at Pepperdine University and the Pepperdine Law School. From June 2003 to February 2005, Mr. Halverson served as President and Chief Executive Officer of the Thomas Kinkade Company. He was Executive Vice President and Chief Operating Officer of Salem Communications Corporation from 1995 to 2000. Prior to becoming Chief Operating Officer he was the company’s Vice President and General Counsel for 10 years. Mr. Halvorson was a partner at Godfrey and Kahn, a law firm based in Milwaukee, Wisconsin from 1976 until 1985. Mr. Halvorson is a Certified Public Accountant and holds a B.S. in Accounting from Bob Jones University and a J.D. from Duke University School of Law. Mr. Halvorson is currently a director of Salem Communications Corporation.
Dick Kramer is President of R.J. Kramer Associates L.L.C., serving as an executive consultant and senior advisor to healthcare companies who provide products and services to integrated hospital systems and medical groups. Clients range from early stage businesses in health and medical technology to Fortune 500 companies serving the healthcare marketplace. Mr. Kramer is Past President and Chief Executive Officer of Catholic Healthcare West, a healthcare system comprising 48 acute care hospitals and affiliated medical groups in Arizona, California and Nevada. He has been recognized for his visionary leadership and execution of strategic growth and business development in competitive managed care markets. In 1997, Mr. Kramer was named runner-up CEO of the Year by the San Francisco Business Times. Mr. Kramer is a graduate of Penn State University, earned a masters degree in counseling from Syracuse University and a masters degree in health care management from the University of Minnesota. He is a graduate of the Harvard Business School Advanced Management Program.
Dr. Levy currently serves as Venture Partner with Frazier Healthcare Ventures, a leading provider of venture and growth equity capital to emerging healthcare companies. He brings more than 30 years of experience in the medical device industry. Prior to joining Frazier Healthcare Ventures, he co-founded and served as President and Chief Executive Officer of Northstar Neuroscience (NASDAQ: NSTR), a company pioneering the development of cortical stimulation therapies. Before that, he served as President and CEO of Heartstream, a company that developed, manufactured and marketed an innovative automatic external defibrillator that revolutionized the treatment of sudden cardiac arrest. Prior to joining Heartstream, he was President of Heart Technology, which developed, manufactured and marketed devices for removing plaque from coronary arteries. Before joining Heart Technology, Levy was Vice President of Research and New Business Development and a member of the Board of the Ethicon division of Johnson & Johnson. Currently, he serves on the Boards of Barosense, Calypso® Medical Technologies, Intuitive Surgical, Proteus Biomedical, Orthocon and Northstar Neuroscience. Levy received his Bachelor’s Degree in Chemistry from City University of New York and his Doctorate in Organic Chemistry from Purdue University.
Floyd D. Loop was the CEO and chairman of the Board of Governors of The Cleveland Clinic Foundation from 1989-2004. Earlier, he chaired the Department of Thoracic and Cardiovascular Surgery at the Cleveland Clinic from 1975 – 1989. Dr. Loop and his colleagues were responsible for today’s widespread use of arterial conduits in coronary artery surgery, innovations in valve repair, reoperations and numerous changes in technical procedure. As a surgeon, Dr. Loop performed more than 12,000 open heart operations and authored 350 papers on all aspects of cardiovascular surgery. Dr. Loop has served as president of the American Association for Thoracic Surgery, Chairman of the Residency Review Committee, and as a member of the American Board of Thoracic Surgery. He has received honorary degrees from Cleveland State University, Purdue University and St. Louis University among many other international awards. He also serves as a director of Tenet Healthcare Corporation, Athersys, Inc. (public), and on the boards of several private corporations.
Mark J. Rubash is presently the Chief Financial Officer of Shutterfly, Inc. Prior to joining Shutterfly, Mark was CFO of Rearden Commerce, and previous to that, Senior Vice President of Finance at Yahoo! Prior to joining Yahoo!, Mark held various senior leadership positions at eBay, Inc. including Chief Accounting Officer and Vice President for Finance and Investor Relations. Prior to eBay, Mark served as the Chief Financial Officer of Critical Path, Inc. and was an audit partner at PricewaterhouseCoopers LLP where he served as the global leader for the firm’s Internet Industry Practice. During this time, he also served as practice leader of the Silicon Valley Software Industry Practice. He earned his Bachelor of Science degree in Accounting from California State University Sacramento and later became a Certified Public Accountant in the state of California. Mark presently serves as chairman of the audit committee and a member of the Board of Directors of Line 6, Inc.
George Stalk is a senior partner and managing director of the Boston Consulting Group and an Adjunct Professor of Strategic Management for the Rotman School of Management at the University of Toronto. He practices strategy development and deployment. Stalk has been with the Boston, Tokyo, Chicago and Toronto offices of BCG.
He is the author of numerous best-selling books including Competing Against Time and Kaisha: The Japanese Corporation, Hardball: Are You Playing to Play or to Playing to Win?and most recently, Strategies in Our Future. Articles by Stalk have been published by many business publications, including Harvard Business Review, where one of his articles won the ‘McKinsey award’ for being the best of its year. Business Week identified George as one among a new generation of leading management gurus. He was also listed as one of the top 10 consultants by Consulting Magazine.
George holds a BS in engineering mechanics from the University of Michigan, an MS in aeronautics and astronautics from Massachusetts Institute of Technology and an MBA from Harvard Business School.
